Worked example: 5 mg from a 30 mg vial
The calculator above starts from a 30 mg Tirzepatide vial reconstituted with 3 mL of bacteriostatic water. That is 10 mg/mL. 5 mg divided by 10 mg/mL is 0.5 mL, and on a U-100 insulin syringe 1 mL is 100 units, so the mark to draw to is 50 units. 5 mg is the second rung of the tirzepatide ladder and a common MOTS-c and epitalon dose. It is also a whole 5 mg vial, so with that vial size it is simply everything you mixed.

- How many units is 5 mg on an insulin syringe?
- It depends on the concentration. At 10 mg/mL (for example 20 mg in 2 mL) 5 mg is 50 units. At 20 mg/mL (60 mg in 3 mL) it is 25 units. At 15 mg/mL (30 mg in 2 mL) it is 33.3 units. At 5 mg/mL (10 mg in 2 mL) it is 100 units. Units are volume; without the vial strength and the water there is no single answer.
- How much bacteriostatic water should I use for a 5 mg dose?
- Whatever puts the draw between about 10 and 50 units, where the barrel is easiest to read. From a 30 mg vial, 3 mL gives 10 mg/mL and puts 5 mg at 50 units. More water means a bigger, easier draw of the same dose; less water means a smaller one. Most vials take 2 to 3 mL comfortably.
- Is 5 mg the same as 50 units?
- Only at 10 mg/mL, which you get from 10 mg in 1 mL, 20 mg in 2 mL or 30 mg in 3 mL. At that concentration units is simply the dose in mg times ten. At any other concentration the number is different: 100 units at 5 mg/mL, for example.
