How to read it

Semaglutide has a licensed ladder: 0.25 mg weekly for four weeks, then 0.5 mg, 1.0 mg, 1.7 mg and 2.4 mg, each held for four weeks before the next step. As with tirzepatide, the first rung is a starting dose for tolerance rather than a maintenance dose.

The doses are small enough that the water volume matters more here than for any other compound. A 5 mg vial in 2 mL puts 0.25 mg at 10 units. The same vial in 1 mL puts it at 5 units, where a one-unit misread is a 20 percent dosing error.

A titration ladder is a maximum pace, not a target. Holding at a lower dose for longer is a normal outcome, not a failure.
