Tesamorelin is a stabilised GHRH analogue and, unusually for this list, a licensed medicine (Egrifta) for HIV-associated abdominal fat, where the licensed dose is 2 mg once daily. Research vials of 2, 5 and 10 mg are sold outside that licence and are not the licensed product. Because the dose is in whole milligrams rather than micrograms, the draw is large compared with most peptides here, and the water volume is what decides whether it fits one syringe.

How many units is 2 mg of tesamorelin?
40 units from a 10 mg vial reconstituted with 2 mL of bacteriostatic water, which is 5 mg/mL and makes 2 mg a 0.4 mL draw. From a 5 mg vial with 1 mL it is also 40 units, because that is the same concentration. From a 2 mg vial with 1 mL it is the whole vial, 100 units.

Frequency and how long a vial lasts
Tesamorelin is a once-daily compound, taken at the same time each day, and the licensed product is given in the evening. At the licensed 2 mg a day a 10 mg vial is five days, a 5 mg vial is two and a half, and a 2 mg vial is a single dose. That is the practical problem with it: the vial-to-dose ratio is poor compared with peptides dosed in micrograms, so a month at 2 mg daily is 60 mg of material.

Water volume, and the 2 mg vial
A 2 mg vial is one licensed dose and nothing else, so the only question is how much water makes it drawable: 0.5 mL puts the whole 2 mg at 50 units, 1 mL puts it at 100 units, the full syringe. A 10 mg vial is the opposite problem, five doses that have to stay stable in the fridge between them. At 2 mL it gives 5 mg/mL and a 40 unit draw, which is the middle of the readable range; at 1 mL it is 10 mg/mL and 20 units, which is tighter but leaves more room if you are splitting the dose.
Half-life is about 26 to 38 minutes after subcutaneous injection, which is why the timing of the dose gets more attention than the exact unit count. The peptide is gone within a couple of hours; what it triggers lasts longer.

- How much bacteriostatic water for a 10 mg tesamorelin vial?
- 2 mL is the usual choice and gives 5 mg/mL, so 1 mg is 20 units and 2 mg is 40 units, both in the easy part of the barrel. With 1 mL those halve to 10 and 20 units.
- How long does a 10 mg tesamorelin vial last?
- Five days at 2 mg once daily, or ten days at 1 mg. A 5 mg vial is two whole doses at 2 mg, and a 2 mg vial is one.
- How often is tesamorelin injected?
- Once daily. The licensed product is given in the evening by subcutaneous injection. Its half-life is about 26 to 38 minutes, so nothing carries over from one day to the next.
- Is research tesamorelin the same as Egrifta?
- No. Egrifta is a licensed medicine supplied at a set strength with its own quality control. Research vials are sold outside that licence, are not approved for human use, and are not interchangeable with it.
