Semaglutide doses are small, which makes this the compound where the water volume matters most. A unit on a U-100 insulin syringe is 0.01 mL, so what a unit is worth in milligrams depends entirely on how concentrated you made the solution. Vials come in 2, 3, 5, 10 and 15 mg. Every answer below names the vial and the water it assumes, because without those two numbers a unit count says nothing at all. Semaglutide is licensed as a pre-filled pen and as a tablet; material sold in vials for reconstitution is not the licensed product and is not interchangeable with it.
How many mg is 40 units of semaglutide?

Semaglutide is dosed once a week, so doses per vial is also weeks of supply. The water volume does not change it.

Whole doses per vial, rounded down, at once-weekly dosing. A 5 mg vial is 20 weeks at the 0.25 mg starting rung and 2 weeks at 2.4 mg. Neither figure allows for the small amount left in the vial or lost in the syringe.
Small doses, small draws
The starting rung of the ladder is 0.25 mg. From a 5 mg vial with 2 mL that is 10 units, which is readable. From the same vial with 1 mL it is 5 units, where being one mark out is a twenty percent error on the dose. This is the one compound on the site where erring towards more water is usually the easier choice: the draw gets bigger, the barrel gets easier to read, and the dose does not change at all. The ceiling is whatever the vial will physically take, which for most stoppered vials is somewhere under 5 mL.

- Why are my units different from someone else's?
- Because they used a different amount of water, a different vial size, or both. A unit count is only meaningful alongside the vial strength and the water volume. Always state all three.
